Job Description

As an immensely successful welsh based organisation with over 20 years experience in delivering specialist mental health and learning disabilities services, MHC are opening a new leading edge Residential Care Home with nursing support in November 2010.

 

Glyn Rhosyn Residential Care Home with Nursing will support 10 men, living in their community, as they move along their own individual care pathways towards more independent living. As well as eight en-suite bedrooms, there will be a two-bedroom self-contained flat where there will be an even greater emphasis on learning independent living skills in preparation for a move into a residential or community service.

 

The service will utilize the unique model of care, known as S.H.A.R.E.D. It is a systematic way of providing Supportive Help Achieving Realistic Effective Development. 

It is based on principles of the nursing process, MDT working, non-aversive strategies, extended behaviour modification, person-centred planning, CPA, the holistic approach and the skills and experience of service and wider organisations staff.  It is a tool to help staff, and the person, make sense of their world and their historical responses to it. It helps them in developing their own strategies, which are consistent with positive progress for each individual.

It utilises a sequential process to understand and discover the person, maintain an optimistic approach, and share challenges / achievements.  Ultimately it is a positive way to unlock the person’s potential, and improve their quality of life. The process is person-centred and encapsulated within the Care Programme Approach.
